Home Care Program

The Center For Hope Hospice & Palliative Care, known for its work with the terminally ill since 1983, is now a licensed home care agency, offering home health services for non-terminally ill patients.

The Center For Hope Home Care offers services to individuals who are homebound, seriously ill or in need of the assistance of visiting health professionals. The hospice experience of the home care team gives them a valuable insight into treating seriously ill patients at home.

Even though home care patients are usually in a recovery phase and have a good prognosis, they may be confronting very complex issues. These issues can include coping with a loss of independence due to physical limitations, the fear of creating stress on other family members and even the inevitability of their own mortality. These are difficult concerns to express, yet part of healing involves recognizing and responding to the emotional as well as the physical effects of an illness.

The Center For Hope Home Care offers:

  • an alternative to institutional care
  • assistance with the transition from hospital/nursing facility to home
  • individuals the opportunity to remain in the home
  • respite for the caregiver

The Center For Hope Home Care team consists of:

  • Registered Nurses
  • Licensed Practical Nurses
  • Certified Home Health Aides
  • Personal Care Aides

Services are available twenty-four hours a day, 7 days a week. Supervision of all personnel is provided by the Director of Nursing.

What Services are Available?

  • Assist with activities for daily living
    - bathing in bed, shower or tub
    - care of teeth, mouth, skin and hair
    - grooming and dressing
    - meal preparation, modified diets
    - assist with feeding patient
    - general household management & cleaning
  • Assist with transfers to bed, chair and commode/toilet
  • Reinforce physical therapy exercises
  • Document care and communicate information to the Director of Nursing
  • Assure patient safety and sense of well being
  • Provide companionship for patient and respite for family and caregivers

Skilled Care Services:

  • Specialized nursing procedures as directed by the patient's physician
    - pain management
    - wound and ostomy care
  • Administration of oral medications and injections
  • Implement and supervise the plan of care
  • Professional support and counseling for the patient and family
    - Social workers
    - Spiritual counselors

What Can I Expect?

The Center For Hope Home Care adheres to a strict code of ethics. The nursing staff is involved with all aspects of patient care and works very closely with the patient's physician to assure projected outcomes are monitored.

In an effort to provide the highest quality of care in the home, all employees at the Center For Hope Home Care are subjected to:

  • Comprehensive skills testing
  • Thorough personal interview
  • Compliance with all state mandated health requirements
  • Verification of all licenses/certificates and references
  • Orientation to all policies and procedures and an annual review process
  • Regularly scheduled in-service education
  • Scheduled employee performance evaluations

Special effort is always made to match the skill and personality of the employee to the needs of the patient.

What Happens Upon Referral?

Upon referral, the Nursing Supervisor will visit the patient in the home, the hospital or skilled nursing facility. An initial assessment is performed, identifying the type of services needed and the category of personnel required to best meet the patient's needs.

An individualized Plan of Care is developed by the patient's attending physician and the Nursing Supervisor for the Center For Hope Home Care. The nursing personnel assigned to each case is oriented to the patient's Plan of Care and works closely with the Nursing Supervisor to implement it effectively. In addition, the Nursing Supervisor will make regularly scheduled visits to monitor the patient's progress.

Fees and Reimbursement
