The Center For Hope Hospice & Palliative Care offers the Community Outreach Program to senior housing entities to address the health and social needs of all senior citizens. The goal of the program is to assist senior citizens with issues that pertain to their physical, emotional and spiritual health and sense of well-being. Through information and supportive services, the individual's independence and the ability to age in place is enhanced.
All services are provided through the interdisciplinary team of The Center
For Hope Hospice. The team consists of nurses, social workers, home health
aides, spiritual counselors and volunteers.
Services include:

an on-site health fair with representation from each discipline to introduce
the program to residents.

a Center For Hope Hospice nurse is scheduled at the senior facility for up
to eight hours per week to monitor vital signs, blood pressure, weight, blood
sugar, etc. Residents have the opportunity to be examined, ask questions
and receive monitoring of their medication plan. If requested, the nurse
will communicate with the individual's attending physician as needed.

follow-up evaluation for residents upon discharge from the hospital to determine
the need for home care.

twenty-four hour on-call nursing service to provide information and guidance.

availability of Center For Hope Hospice & Palliative Care staff members from each discipline
to address concerns relating to their area of expertise. The following team
members are available upon request in addition to scheduled visits from a
nurse.
Social worker: to answer questions regarding Medicare, Medicaid,
Advance Directives, and financial issues related to health care.
Spiritual counselor: to address issues of loss, bereavement/grief,
and death & dying.
Volunteer: to provide companionship, assistance with shopping or
transportation when usual methods have been exhausted.
